Activity on Referral


What is the Scheme?

The Activity on Referral (AoR) scheme is a countywide initiative whereby health professionals can refer physically inactive people with a medical condition to their local leisure facility. The scheme helps individuals to become more active by providing them with support and a tailor-made exercise programme.

During the 12-week programme, participants are encouraged to build up their weekly activity levels by attending the supervised sessions at the centre of their choice. Participants can attend as frequently as they wish and will be offered a range of options including gym, swim and classes according to their preference and the venue.

The exercise professional will conduct a consultation at the beginning of the programme and again at the end of the 12-weeks to assess improvements in the participants’ health and wellbeing. On completion, participants will be encouraged to continue with regular physical activity independently and will be provided with information on local activities including the option to join the facility as a member.

Who is eligible?

To be eligible for the scheme the individual must:

  • Be over 16 years
  • Inactive
  • Suffer from a medical condition that would benefit from a programme of exercise, including the following eligible conditions;
  • Coronary heart disease risk factors (eg: hypertension, high cholesterol
  • Pre-diabetes / type 1 or type 2 diabetes
  • Stroke
  • Depression, anxiety, stress or other mental health problems
  • Asthma or Chronic Obstructive Pulmonary Disease
  • Obesity / Overweight
  • Impaired strength or mobility / osteoarthritis / osteoporosis
  • Neurological conditions
  • Musculo-skeletal problems, such as joint or back problems
  • Degenerative diseases

How to make a referral?

Referrals are made by completing the Activity on Referral Form found on SystmOne. A PDF of the referral form is also attached.

Health professionals will need to ensure a copy of the referral form is given to the patient so they can take it along with them to the initial consultation. A copy of the referral should be kept on the patient’s medical records.

Adults Weight Management

What is the scheme?

Adult lifestyle weight management services offer 12 week programmes for overweight or obese adults across Northamptonshire. These programmes offer weekly or fortnightly sessions that are multi-component; addressing diet and nutrition, physical activity levels, and behaviour change techniques.

Programmes are designed to suit individuals’ lifestyle and needs and will ensure that achievable goals for weight loss are set. During the 12 weeks,

Weight management services are provided by selected partner providers, with meetings taking place at a number of locations across the county. These include:

  1. Slimming World (county-wide)
  2. Weight Watchers (county-wide)
  3. Wellness, Weight Management and Lifestyle Centre (Kettering)
  4. Trilogy Leisure (Northampton)
  5. Northampton Town Football Club (Northampton)

Who is eligible?

To be eligible for this service the individual must:

  • Have BMI ≥ 28
  • Have not paid to attend at Slimming World or Weight Watchers in the last 3 months.
  • Be resident in Northamptonshire
  • not be pregnant
  • be aged 18 years or over
  • be ready and motivated to lose weight
