Diabetes Prevention Programme (DPP) - now sending discharge notices to GP's

Dear Colleagues.

Firstly many thanks for all your efforts in identifying and referring patients with Pre-Diabetes to the Diabetes Prevention Programme (DPP).

We appreciate that feedback to clinicians has been limited regarding patients referred and so the provider has agreed to send correspondence at the point of patient discharge from the service.  Up until recently the provider has been keeping patients on the list if they do not initially take up the invite to attend in order to offer further chances.  However they have now formalised a policy whereby patients will be offered a set number of chances before being discharged.

As there will be a backlog of patients there may be numerous letters being sent to you. This is for information only to update your clinical records and no specific action is required.
